Client Responsibilities
- a. Provide accurate and complete personal and medical details during the intake process.
- b. Attend all scheduled appointments punctually and adhere to our cancellation policy.
- c. Actively participate in the treatment plan, follow clinical recommendations, and communicate any concerns or changes regarding your condition.
- d. Make timely payments for services in accordance with our fee schedule and payment policies.
Provider Responsibilities

- a. Client information, including medical and personal records, will remain confidential and will not be shared with third parties without written client consent except where legally mandated.
- b. The Provider will implement appropriate security measures to safeguard client information.
Termination of Services
- a. Either party may discontinue services by submitting a written notice.
- b. The Provider reserves the right to terminate services if the client engages in conduct that endangers themselves or others or fails to comply with these Terms.
Intellectual Property
- a. Any materials provided by the Provider, including treatment plans, therapy documentation, and educational resources, are legally protected intellectual property.
- b. Clients may not copy, distribute, or alter these materials without obtaining prior written consent from the Provider.
Limitations of Liability
- a. The Provider’s liability for claims related to the services is limited to the amount paid by the client for the specific services rendered.
- b. The Provider is not responsible for any indirect, incidental, or consequential damages resulting from the use or inability to use our services.
Governing Law and Jurisdiction
These Terms are governed by the laws of Maryland. Any disputes arising from these Terms shall be resolved exclusively in the courts of Maryland.
